Hospice Nurse Practitioner – PRN

How You’ll Make a Difference:

✅ Provide expert in-home care to prevent unnecessary hospitalizations
✅ Assess, diagnose, and manage hospice and palliative care patients
✅ Educate and support families through end-of-life care decisions
✅ Maintain accurate patient records to ensure quality, compliant care
✅ Collaborate with the care team (Medical Director, attending physician, IDG, and branch leadership)
✅ Help develop clinical practice guidelines to enhance quality palliative care

What You Will Need:

✔ Master’s Degree in Nursing with Nurse Practitioner certification
✔ 3+ years of clinical nursing experience (1+ year in hospice/palliative care preferred)
✔ Experience in Family Medicine, Geriatrics, or Psychiatry preferred
✔ Active state Nurse Practitioner license & DEA license
✔ Board Certification in Palliative Care preferred
✔ Willingness to travel up to 50% as needed
