Position Details:

Schedule: Full-time, Monday to Friday, 8:00 am to 5:00 pm

Coverage Area: Portage, Delton, and Battle Creek

Your Key Responsibilities:

Holistic Patient Assessments: Evaluate the physical, emotional, and environmental needs of hospice patients, ensuring they receive the best possible care.

Tailored Care Plans: Create and implement personalized care plans to meet each patient's unique needs.

Ongoing Evaluation: Perform regular follow-up visits, adjusting care plans as needed to maintain the highest standards of care.

Effective Communication: Keep the care team informed of significant changes and findings regarding patient health.

Patient & Family Education: Provide guidance on hospice philosophy, pain management, and support at the end of life.

Documentation: Maintain accurate, thorough records that align with HIPAA regulations.

Act as an advocate and resource, ensuring patients’ needs are met with dignity and respect.

What We’re Looking For:

A graduate from an accredited nursing program

Current RN licensure in the state.

At least 1 year of experience in a clinical setting (hospice or home health experience preferred).

Strong nursing skills aligned with established standards of care.

Reliable transportation to reach patients in the designated coverage area.

Excellent organizational skills and the ability to manage multiple priorities.

Preferred to live in service area (less driving for you and better response time for patients!)
