Registered Nurse-Liaison-Hospice-Intake- (Capital Region-primarily Samaritan Hospital, Troy, NY)

The Community Hospice has a rewarding opportunity for a RN to work as a part of our Hospice Intake RN Team serving as Liaisons in local hospitals.

What you will do:

Assessments, care coordination, and collaborationEvaluate hospital referrals for hospice servicesDescribe the support and care available from hospice along with the benefits of hospice care.Admission of patients to services in the hospital if applicableCoordinate with the patient’s community physician and our hospice medical directors to confirm eligibility for hospice services.Gather information and ensure a smooth handoff to the hospice RN admitting the patient/family to services.Work closely with the hospital case management department.The work schedule is M-F either 8a-4:30p or 9a-5:30p


What you will need:

A current license to practice as a Registered Nurse in the State of New YorkAssociates Degree in Nursing, BSN preferred1 year experience as an RN , preferably in Hospice/Home Care or acute care.The RN must be able to communicate effectively, verbally and writtenValid Driver's License and Reliable vehicleHigh level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkersUnderstanding of Hospice principles preferred


Training provided in end of life care.

Pay Range:$33.10 - $45.75

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
