About the Role
As a Medical Social Worker with Elara Caring, you will:

Deliver patient-centered hospice social work that aligns with our mission of delivering the right care, at the right time, in the right place.

Collaborate with an interdisciplinary team to develop and execute care plans specific to each patient’s needs.

Provide 1:1 support and care tailored to patients’ unique social, emotional, and psychological needs, helping them and their families navigate the challenges of hospice care.

What You’ll Need to Succeed

Master’s or Doctoral degree in Social Work from a CSWE-accredited program

Current state Social Work licensure

At least 1 year of social work experience in a healthcare setting

Strong social work skills with adherence to professional standards

Reliable transportation, valid driver’s license, and willingness to travel within a 50% service area
