Hospice Social Worker (LiCSW/LCSW)

Schedule: Monday through Friday, from 8:00a to 5:00p

Your Impact as a Hospice Social Worker

In this vital role, you’ll:

Conduct assessments to identify patients’ psychosocial, financial, and environmental needs.

Develop and implement individualized care plans that prioritize safety and well-being.

Collaborate with healthcare teams, patients, and families to ensure continuity of care.

Provide counseling and support to patients and their loved ones.

Monitor and adjust care plans as needed, always maintaining high standards of practice.

Advocate for patient needs by coordinating resources and ensuring compliance with care goals.

Supervise and mentor other social workers on staff, providing guidance and ensuring quality care.

What You Bring

To excel in this role, you’ll need:

A Master’s or Doctoral Degree in Social Work from a CSWE-accredited program.

A current State License as a Social Worker as a LiCSW or a LCSW

At least 1 year of social work experience in a healthcare setting.

Exceptional verbal and written communication skills.

A compassionate and proactive approach to patient care.
