In support of our ongoing growth we are seeking highly skilled and motivated Advanced Practice Providers to join the regions only Hospital At Home Team.
Key responsibilities:
+ Conduct Telemedicine visits and Inpatient rounding at both the Downtown and Community Campus, providing education to and interfacing with primary teams, patients, and families prior to transferring to the home setting.
+ Assess and evaluate patients, order and interpret laboratory tests & diagnostic studies, and arrange referrals, consultations, and therapeutic services.
+ Build and maintain strong relationships with patients, families, referring physicians, and all members of the health care team.
As an APP, you will be an integral member of the multidisciplinary healthcare team, demonstrating professionalism, leadership, and a commitment to Upstate’s mission, vision, and values, while adhering to all institutional policies and procedures.
Minimum Qualifications:
+ 1 year or more of experience in role in acute care medicine
+ Master’s Degree as a Nurse Practitioner or Physician Assistant.
+ NYS Licensed/Registered NP or PA.
+ Current National Certification consistent to the clinical practice.
+ NYS licensed/registered NP or PA required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.
+ CPR-BLS Certification required at the time of appointment.
+ Must successfully pass pre-employment and credentialing clearances.
Preferred Qualifications:
+ Experience in home health, community health, or public health.
Work Days:
Monday - Friday, Days, with possible weekend and holiday rotation.
