Hospital DME Liaison

With the support of more than 30,000 team members, we promote a forward-thinking philosophy that’s built on a foundation of trust, dignity, respect, responsibility and clinical excellence. **The Hospital DME Liaison Responsibilities Include:** + Acting as the main point of contact for hospital staff who are discharging patients home with DME (Durable Medical Equipment) and home oxygen. + Communicating with Care Coordination and hospital staff to facilitate the timely delivery of DME to patients prior to discharge within the Cerner, HC+ and other Electronic Medical Record Platforms. + Communicating with outside DME companies (as needed) to coordinate the necessary paperwork and delivery of DME. + Collaborating with Customer Service to facilitate timely delivery of DME.
