Summary:
Main point of contact for hospital staff who are discharging patients home with DME (Durable Medical Equipment) and home oxygen. The HomeCare DME Liaison will communicate with Care Coordination and hospital staff to facilitate the timely delivery of DME to patients prior to discharge within the Cerner, HC+ and other Electronic Medical Record Platforms. HDL’s communicate with outside DME companies (as needed) to coordinate the necessary paperwork and delivery of DME. HDL's will collaborate with Customer Service to facilitate timely delivery of DME. This process is meant to reduce readmissions and promote a seamless and timely discharge of patients. HDL's will be responsible for inventory management of hospital DME closet.
