The Hospital DME Liaison Responsibilities Include:
Acting as the main point of contact for hospital staff who are discharging patients home with DME (Durable Medical Equipment) and home oxygen. Communicating with Care Coordination and hospital staff to facilitate the timely delivery of DME to patients prior to discharge within the Cerner, HC+ and other Electronic Medical Record Platforms. Communicating with outside DME companies (as needed) to coordinate the necessary paperwork and delivery of DME.Collaborating with Customer Service to facilitate timely delivery of DME. This process is meant to reduce readmissions and promote a seamless and timely discharge of patients. Inventory management of hospital DME closet.Position details:

Required High School Graduate or Equivalent GEDExperience:
Required 1 year Health Care Environment or 2 years Customer Service or 6 months Cerner and HC+Associates Degree in lieu of experienceCertifications and Licensures:
