Hospital RN Care Manager  

 Responsibilities

         Performs the initial comprehensive assessment on admission         Provides ongoing assessment of needs          Monitor effectiveness of services          Track progress toward individual goals          overseeing transitions for patients and assess all discharge planning needs

Qualifications

         BSN required         New York RN license/registration.          One year of Hospital Care Management experience          Care Management Certification          PRI Certification
