Position Summary:
Responsible for providing prevention and education services in a variety of settings including on the phone. May implement prevention services over a varied group of topics including substance abuse, mental health, bullying, suicide and other areas as necessary.

(2) First Shift (7am - 3:30pm/Fri, Sat, Sun) Some holidays
(1) Second Shift (3:30pm - 11:30pm/ Fri, Sat, Sun) Some holidays
(1) Third Shift (11:30pm - 7:30am/Fri, Sat, Sun) Some holidays
Position Description:
Identifies and assesses needs of callers/clientsProvide crisis intervention, referrals and linkage to community resourcesUtilizes supportive techniques and/or educational methods that are appropriate to caller and topicCompletes service documentation accurately and timely in electronic health record Provides both prevention and education servicesFacilitates referral to and/or provides appropriate level of careDevelop, define and implement realistic and effective goals and plans for interventionsCollect, track and evaluate program data for fidelity and service qualityAdhere to professional standards as outlined by protocols, rules and regulationsOther duties as assignedThis is not a remote positionRequired Knowledge, Skills and Abilities:

High School diploma or GEDPrefer experience in social services environment or related educationEssential Functions/Physical Demands:
