Position Summary
The House Monitor for the Booth Home Senior Bridge Housing program provides general oversight of residents and their rooms, assists with resident check-ins and discharges, and evaluates and reports on residents’ conditions to the Case Manager. Additionally, the House Monitor is responsible for completing documentation, tracking residents' progress upon discharge, and conducting follow-ups. The Booth Home Senior Bridge Housing program serves 15 male and female clients with mental health diagnoses. This position requires working Saturdays and Sundays from 7:00 AM to 3:00 PM.
Essential Functions
Provide professional support and compassionate, safe care for all residents. Respond promptly and courteously to clients’ needs. Perform all duties in a respectful manner that upholds clients' and staff members' sense of self-worth. Monitor clients to ensure a safe, quiet, and friendly atmosphere is maintained, including making periodic rounds of the house. Clearly explain rules and procedures to all clients and ensure respectful adherence to policies. Complete all intake forms and records, establishing individual resident files for new intakes. Adhere to all Booth Home Senior Bridge Housing and The Salvation Army policies and procedures. Assist the Case Manager with data entry and documentation, including but not limited to intake forms, maintaining resident files, and discharge documentation. Complete all daily logs, obtain necessary signatures, and provide thorough end-of-shift reports to the oncoming shift and Case Manager. Document and report any unusual occurrences, incidents, or injuries. Ensure common areas and hallways remain free of obstructions and safety hazards. Work collaboratively with staff to uphold safety and security measures. Maintain security inside and outside the building. Respond promptly and appropriately to emergency situations. Always uphold resident confidentiality. Assist the Case Manager in documentation and tracking of residents upon discharge. Answer and direct incoming calls to the appropriate individuals in a courteous and professional manner. Assist with meal service as needed, including breakfast, lunch, and dinner; handle cleanup after meal service and comply with all food service and health codes. Help maintain cleanliness of common areas, manage room turnovers, handle house laundry, and sort donations. Be available to work an irregular schedule with varying hours as needed. Perform other duties as assigned by your immediate supervisor and/or management personnel.Working Conditions
Ability to walk, stand, bend, squat, climb, kneel and twist on an intermittent or sometimes continuous basis. Ability to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead. Ability to operate computer, fax and telephone. Ability to lift up to 25 lbs.
Minimum Qualifications
A criminal background check is required with certificate for Protect the Mission policies and procedures. High School Diploma or GED Required, A.A. or B.A. /B.S. highly preferred. Knowledge and sensitivity regarding issues of homelessness, mental health and chronic illness Complete required Salvation Army trainings (Child Safety (PTM), etc.) Ability, both visual and auditory, to identify safety hazards and monitor guest and staff behavior and enforce appropriate safety regulations and emergency procedures Must possess flexibility, effective leadership, conflict resolution, and communication skills.
