Housing Coordinator (4271HC)

General Statement of Duties: Work with Persons enrolled on the Integrated Team/Supervised GLEs to facilitate movement into permanent Independent Housing and maintain tenancy.

Responsibilities:

1. Utilize recovery principals in partnership with Persons Served to improve Persons’ health and wellness; achieve symptom stabilization and self-management; develop and/or restore skills impacted by their mental illness; live a self-directed life; and strive to reach their full potential.

2. Utilize the core principles of Housing First to plan and deliver housing services.

3. Support Persons with assessing and maintaining affordable permanent housing. Provide interventions to Persons in all housing settings to maintain tenancy, support housing transitions and prevent homelessness.

4. Develop housing opportunities and options which align to the age-related and support needs of Persons.

5. Provide skill building opportunities which focus on engagement and problem solving as primary methods for developing relationships, establishing treatment goals, and minimizing restrictions/barriers to housing.

6. Assist in the implementation of Treatment Plan interventions. Complete required service notes daily.

7. Coordinate treatment and services with all Persons’ Served Care Coordination Entities and behavioral and physical health providers.

8. Work directly with the Person Served to obtain and maintain documentation related to Person’s housing search and/or subsidy process including Mass ID, birth certificate, income statements, for each person served as applicable.

9. Manage the DMH Rental Subsidy Program (DMHRSP) required documentation. Act as primary contact for the local Housing Authorities and the DMH Licensor for securing new units and to comply with annual requirements.

10. Facilitate quarterly inspections for all agency leased apartments on the ACCS Integrated Team, notify Program Director of safety related concerns. Participate as needed with communicating concerns to landlords, property managers, etc. and enabling corrective action.

11. Participate in Housing First meetings at DMH; attend housing-related training as needed.

12. Act as Team liaison to Mainspring, Father Bills, and landlords. Collaborate with The Project for Assistance in Transition from Homelessness (PATH) and Housing Solutions for Southeast Mass Programs as needed.

13. Oversee monthly Housing Rounds meeting for the Team to discuss Person related housing needs.

14. Act efficiently to preserve tenancy/facilitate rapid access to housing when Person’s tenancy issues arise. Support individuals at court appearances related to housing issues and assist them in resolving concerns.

15. Collaborate with the team to ensure the protection of individuals’ tenancy related rights and help them understand the Community Residency Tenancy Law (CRT) at the Supervised GLEs, grievance procedures and providing support to file, mediate, and resolve complaints.

16. Follow funder and agency protocol for reporting significant incidents and alleged abuse and/or neglect.

17. As a representative of BAMSI, always communicate in a professional and respectful manner.

18. Execute duties to reflect safety standards. Universal precautions must always be utilized.

19. Transport individuals served as needed.

20. Understand and utilize universal/standard precautions and instructions on how to stay safe in areas that constitute risk, including screening for suicide risk and following persons served Risk and Safety Plans as applicable.

21. Obtain required training within allotted time frames.

Qualifications:

· High school diploma or equivalency required, associate or bachelor’s degree in human services or related field preferred.

· Experience related to managing affordable housing and/or housing supports.

· Experience working with people who have been diagnosed with a serious and persistent mental illness.

· Basic computer literacy, including the ability to record data, use electronic medical records, use electronic time and attendance software, and receive and send electronic mail, is required.

· Effective listening, critical thinking, writing, and communication skills.

· Must have valid driver’s license in state of residence and have own means of transportation.

· Preference given to candidates with fluency in languages prevalent in Southeastern MA
