The Housing Navigator position provides assistance, information, and support to clients around securing safe, appropriate housing using subsidies and/or other available resources in New York City that suit the individual’s needs and to maintain residential stability. The Housing Navigator must be aware of and understand current housing subsidies, public benefit programs, and develop resources to include establishing relationships with landlords and brokers.
ESSENTIAL FUNCTIONS AND KEY ACCOUNTABILITIES
Assess each client's housing needs within 48 hours of admission and participate to develop discharge plan
Assist in determination of client’s eligibility for government-financed housing programs and benefit programs
Identify, obtain, and assist documents needed for applications for supportive and permanent housing
Assist in the escorting of clients to housing appointments and referring entities outside of the facility
Identify and maintain resources of key information and individuals in various agencies (HRA, NYCHA, HPD) to facilitate the most current and accurate permanent housing application processes
Develop and maintain a database of available housing resources as well as other relevant community resources (i.e., banks, utility company contacts, grocery stores, pharmacies, transportation, etc.)
Identify new housing opportunities for clients through ongoing networking
Maintain strong working relationships with landlord and management companies
Prepare clients for permanent housing interviews and arranges for transportation or escorts as needed
Ensure that discharge plan meets the safety, self-sufficiently and related need of the client in collaboration with social service staff members
