JOB SUMMARY
Recommend and assist access to medical, educational, social, vocational, housing, and more to support program goals. Initiate referrals to next level care, coordinate with physical health providers for whole-person care, and advocate in the best interest of clients.
RESPONSIBILITIES

Locates housing opportunities in defined areas of the County that comprise the housing continuum including short-term, transitional and permanent options.Coordinates and oversees various kinds of oversight agreements, including master leasing, scattered site leasing, and subsidy applications. Updates team on a regular basis about housing alternatives and changes in availability.Works closely with program direct service staff to coordinate appropriate housing options for clients relative to functioning level and individual needs and preferences.Conducts extensive liaison with professional members of the housing industry as well as homeless and housing providers.Provides information about the program to community agencies connected to the mental health services system in order to educate them about the program.Provides clinical interventions as it relates to housing needs, assists clients in obtaining necessary documents for housing placement, and document client contact using program electronic health record.Works directly with Housing Coordinator to address clients housing needs.Maintains a minimum degree of training and knowledge in cultural competency, co-occurring and gender responsiveness and routinely demonstrates these traits in every day work relationships.Displays regular and reliable attendance. Reports to and departs from work on time, as scheduled, and accurately reflects all time on time card.Successfully functions as a team member.Aids in general management of center including running errands as assigned.Completes all assigned duties within the time frame and according to the standards as established by the Program Manager.Complies with all MHS and program rules, policies and procedures including HIPAA standards.Demonstrates commitment to professional growth by attending in-service trainings and staff development programs, and seeking out supervision when needed.Demonstrates adequate care and responsibility for equipment. Reports and/or repairs missing or damaged equipment and supplies.Ensures facilities upkeep and safety by maintaining neat and clean work areas, by securing work area at close of business day, and by reporting vandalism, theft, or suspicion of drug abuse by coworkers to supervisor as soon as possible.Familiar with and understands Mental Health Systems, Inc. Personnel Policy manual and follows all established personnel policies.MHS requires all employees to attain a minimum degree of training and knowledge in cultural competency, co-occurring andgender responsiveness and to routinely demonstrate these traits in every day work relationships.Participates in regularly scheduled staff meetings.Participates in the Injury and Illness Prevention Program and follows guidelines that promote workplace safety.Reads memos, directives, meeting minutes, and revised policies and procedures as posted.Other relevant duties as assigned.QUALIFICATIONS
For Housing two years of experience and an AA degree in social service or related field or High School Diploma and 5 years’ experience.Valid CA driver’s license and current auto insurance per contract requirements required.Experience providing mental health and/or substance use disorder services preferred.Cultural competency understanding required.Bilingual preferred.Knowledge of Microsoft Office and EHR software preferred.PHYSICAL REQUIREMENTS
