HR Business Partner - Palliative Care Division

This position will directly report to the AVP HR Business Partners Palliative Care & AIM and is responsible for helping to align HR and business objectives with employees and management for Gentiva Health’s rapidly growing palliative division. **This is a work from home position. Our palliative branch locations (Empatia and Illumia Health) are located throughout specific states. To support operational needs and business hours, candidates should reside in** **these states: Georgia, Michigan, North Carolina, Pennsylvania, Tennessee or Texas.** As we continue to quickly launch new Illumia Health Advanced Illness Management (AIM) locations and increase hiring/staffing of field-based clinicians, we are looking for a hands-on HR Business Partner that helps to deliver a seamless end to end employee experience, working across the HR function to deliver value-added service to internal customers and associates. This role is responsible for helping anticipate people-related needs, communicating needs proactively, employee relations, performance management, training, and development, and supporting associates in our growing palliative division. This position works with HR Shared Services to provide a white glove approach for our associates. **Essential Functions of Position** + Provide Human Resources support to new and current associates; providing assistance, policy and procedure questions, sharing relevant HR practices, hosting new hire HR orientation, help welcome new hires and work with the HR Shared Services contacts when appropriate (Payroll, Benefits, Safety, HR Operations, Leave Management, Talent Acquisition, etc.) + Provide support to people leaders on a variety of HR areas, including training, employee relations, performance management, onboarding, HR best practice and company initiatives to ensure positive and complaint associate relations. Share relevant company information to associates or people leaders. + Serve as trusted HR partner for employee relation matters. Partner with branch managers in the investigation and resolution of employee relations and/or performance management issues, including conducting thorough investigations, documentation and conflict resolution. + Provide guidance and assistance to people leaders on performance management (e.g., coaching, counseling forms, development, disciplinary action steps) + Facilitate and deliver new trainings on leadership development and HR-related topics, company practices and systems to help educate people leaders on HR processes, best practices and other company or training initiatives. + Review and utilize HRIS reports/systems to identify trends that can help deliver and drive improvements for retention and engagement (e.g. exit interviews, surveys, turnover reports) + Serve as subject matter expert in HR-related laws, company policies and procedures, providing support to associates at all levels + Serve as an ambassador on behalf of the One HR team, partnering on compliance and best practices in a variety of areas that may include onboarding, talent acquisition, total rewards, learning and development. + Work with HR Processing Specialists as needed to help provide a smooth HR onboarding experience for new hires and hiring manager. + Help monitor annual performance evaluations with the AVP, HR to ensure associate performance reviews are conducted timely. + Build and maintain strong relationships with people leaders and associates. + Other duties as assigned. **About You** **Specialized Knowledge/Skills:** + Well organized and self-motivated to work independently and as part of a team (in a fast paced and remote environment). + Expertise in employment related laws and regulations (state and federal) required. + Excellent oral and written communication skills to communicate effectively with all levels of associates, including field based providers and people leaders. Must be comfortable speaking and presenting for + Exercises professional judgment and demonstrates critical thinking skills, including protecting and maintaining confidentiality. + Familiarity with employment-related state and federal regulations preferred + Advanced proficiency working with technology and electronic applications including HRIS systems, Microsoft Office Applications (Power Point, Excel, Outlook) and other business intelligence systems. + Maintains a strong customer-service focus and commitment. + Flexibility andability to shift work priorities with little or no notice based on business needs. + Approximate percent of time required to travel: 5% **Education/Experience:** + Bachelor's Degree in Human Resources or related field, or a minimum 8+ years' relevant HR experience. + Minimum of 3-5+ years experience in similar human resource business partner role.
