Hub Access Coordinator

Job Description:

Under general direction, the Patient Connection Hub (PCH) Access Coordinator will actively monitor and address all referral and appointment requests using general clerical and customer service skills including, but not limited to, organizing requests from multiple locations and communicating status of appointments/referrals.  The PCH Access Coordinator serves as the point of contact for patients, referring physicians and their office staff, and the various Baptist Health System departments for all access related requests. The goal is to deliver a high-quality and integrated Baptist experience; ultimately ensuring that patients receive the correct care, from the appropriate provider in right timeframe.

Minimum Education, Training, and Experience Required

High School Diploma or equivalent.Strong computer skills.Strong research and problem-solving abilities.Must be able to successfully pass a thorough background check and drug screen.Excellent attendance, punctuality, and ability to meet and maintain established metrics are crucial to being successful in this role.
