HUB Support Associate
Overview The Hub Support Associate works collaboratively with the Manager’s of Care Coordination to manage and prioritize current and near-term (1-2 days) discharges. Responsible for coordination of insurance authorizations for post care, durable medical equipment, transports and other tasks as delegated. (ED as needed) Job Details Education: High School diploma or equivalent required Bachelor’s degree in health-related field preferred Skills: Knowledge of Outlook, Word, Excel, Database and Power Point required; must be flexible and able to work in stressful environment. Must demonstrate the following: Excellent interpersonal skills to effectively communicate with all clinical and service area departments. Proficiency with computer systems required for Hub and areas supported. Ability to determine appropriate sequencing of multiple requests. Ability to manage and prioritize multiple activities while working in a fast paced environment. Must utilize critical thinking and creative problem solving skills in performance of responsibilities. Duties/Responsibilities: Prioritizes the patient’s needs for post-acute care and those of the Managers of Care Coordination. Assists with patient preparedness by entering/monitoring/communicating authorizations. Assists with meeting departmental performance metrics. Centrally prioritizes inpatient service demand, collaborating with MCC’s as needed; ensures inpatients with discharge orders have high priority. Works cooperatively with MCC’s, Nursing, Service Area Department Resources, and Central Scheduling to coordinate the patient itinerary; continually re-assesses and adjusts schedules as necessary. Collaboratively manages the transport of patient requirements with central transport HUB. Collaborating with MCC’s and Supervisor’s of Care Coordination to address barriers to patient progression Manages daily work queue for patients; adjusts request priorities and escalates issues as appropriate to leadership. Salary Range USD $22.46 - USD $27.18 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
