We’re looking for a Human Services Case Manager to work as part of our growing Community & Employment Partners Team supporting the Harford County Region. As a Human Services Case Manager you will work independently in coordination with your regional team to support individuals with neurodiversity to work toward personalized goals by utilizing and exploring their community’s resources. Abilities Network puts an emphasis on community based participation, development and integration. The starting rate of pay for this position is $19.00 per hour and is non-negotiable. The Human Services Case Manager position is also eligible for a $20 per pay auto allowance and mileage reimbursement.

Guide the team to support individuals through the process of setting and working on goals.Provide clinical oversight and leadership to your team by monitoring progress and service delivery.Assist individuals in working toward personal independence by supporting the acquisition and incorporation of new skills and building a system of natural supports.Review goal progress with individuals receiving services, assessing satisfaction, gathering feedback and exploring options for future growth and independence.Driving is required for this position - your own personal vehicle, car insurance and valid driver’s license is required.Deliver customized services tailored to each individual's preferences and personal goals.Provide on-the-job coaching to assist individuals in becoming independent on the job.Encourage the development of individual's interpersonal skills. Required Education and Experience
Degree in Human Services related discipline preferred.High school diploma or GED required.Minimum of 2 years’ experience in the human services field preferred; commensurate experience of 4-6 years in field accepted in lieu of degree.Must have valid driver’s license and positive driving record with access to daily use of personal vehicle to travel within assigned region.Excellent understanding of time management.Strong ability to think creatively and develop solid internal and external relationships with resources in the community.Find out more about us and apply at www.abilitiesnetwork.org.
