MINIMUM QUALIFICATIONS:
+ Current Florida Department of Health (DOH) employee.
+ Willing to work with infectious disease patients (STDs/HIV/TB).
+ Bachelor’s Degree in social science or health discipline from an accredited college OR a Bachelor’s Degree in any other discipline with at least 6 months of direct case management experience or a Florida Licensed Registered Nurse (RN) with 1-year of direct case management experience or a Master’s Degree in Social Science or Health discipline (which can substitute 6 months of direct case management experience.
+ Completion of Medical Case Management AETC Southeast AIDS Education course within 30 days of hire.
+ At least 6 months verifiable experience working with electronic medical records systems.
+ Successful completion of HIV 500/501 course within 6 months of hire.
+ Willing to perform home visits for individuals from diverse backgrounds (varying cultural backgrounds, socioeconomic statuses, and different sexual orientations).
+ Ability to bend, kneel, kneel, use fine motor skills, lift and carry a maximum of 25 pounds .
+ Valid FL Driver’s License and access to a personal vehicle.
+ Emergency Duty .

JOB DUTIES AND RESPONSIBILITIES:
This is a highly responsible position in the Ryan White HIV/AIDS Program (RWHAP) at the Sunshine Care Center (SCC) of the Florida Department of Health in Orange County. The incumbent serves as the Intensive Case Manager for clients accessing services directly at the SCC clinic, and from other public agencies who refer clients for HIV services. Incumbent works in partnership with partner Counties, and other AIDS Service Organizations (ASO’s), including Community Based Organizations (CBO’s). Incumbent reports directly to the Supervisor of Case Management.
Staff will establish and maintain a caseload of no more than 35 clients and updated case information for each client monthly, keep all records in accordance with the local Service Standards and the confidentiality and security protocols and procedures of the DOH-Orange and SCC. Staff will provide at a minimum five (5) client services daily per provider schedule and accurately complete and scan documents into the State and RWHAP systems, Health Management System (HMS) and Provide. Staff, in coordination with SCC Referral Specialists, Peers, Patient Navigators, AIDS Drug Assistance Program (ADAP), Early Intervention Specialist (EIS) and other stakeholder units, will ensure RWHAP Eligibility determination in accordance with most current Florida HIV procedure manual and funding source requirements for the Eligible Metropolitan Area (EMA) and other public agencies that refer clients for HIV services at SCC. Staff will accurately complete, maintain, and update all documentation related to case management.
Staff will ensure the Acuity Assessment is accurately completed and documented within 7 days of the initial client contact. Staff will complete the Client Assessment and Plan of Care within 30 days of initial client contact in an interactive face-to-face interview and monitor progress and sustainability of the Plan of Care through weekly contact with client as determined by the Intensive Case Management Service Standards (SS) and current programmatic policies. Contact must be made to clients regarding appointment reminders and adherence to care. Contact may be face-to-face, by telephone, mail, e-mail or home visits as needed. While not virally suppressed, minimum standard is 4 contacts/month with at least 2 being face ‐ to ‐ face and at least 1 of the face-to-face encounters must be a home visit. Upon viral suppression 3 contacts/month with at least 1 being face ‐ to ‐ face and it must be a home visit. Staff is expected to bill a minimum of 540 units each month.
Staff will provide regular follow up with client’s treatment adherence, medical care, detox, substance abuse, mental health treatment, legal services, and the ability to live independently. Staff will provide HIV Health Education & Risk Reduction services and advocacy to address medical and treatment adherence, psychosocial needs and paths to access resources. Staff will assist client with determining provider preference and arrange for delivery of coordinated services to meet client’s needs and Service Standards in accordance with all RWHAP. Staff must connect clients to culturally sensitive treatment adherence medical care, support services, and collaborate closely with individuals living with HIV to ensure they receive adequate medical and support services.
Staff will document all client contact activities in HMS and Provide within 72 business hours of the activity date. This includes any necessary entries into additional databases or systems, following established guidelines for case management reimbursement, and ensuring compliance with Client Service Records (CSR), Employee Activity Records (EARS), Ryan White Program Services Report (RSR), Health Resources and Services Administration (HRSA) Quality Measures, and other required reports.
Staff must maintain subject matter expertise for assigned duties and participate in all training activities and attend meetings as required by the funding source and Program Manager. Staff shall establish monthly case conferences with clients’ medical and other providers to sustain retention in care and improve the client’s quality of life as needed.
Staff will collaborate effectively with internal and external partners, keeping them informed with necessary and accurate information, while promoting teamwork across all internal and external departments. Staff will produce reports, correspondence, and other work products that achieve the intended objectives.
