HYBRID Community Based Social Worker
The Medical Social Worker collaborates with themultidisciplinary team to provide input in the development of the plan of carefor those patients requiring social work intervention. The Medical Social Worker provides medicallynecessary intervention to patients who have social or emotional problems thatare, or are expected to be, impediment(s) to the effective treatment of themedical condition and/or rate of recovery. Additionally, the Medical Social Worker provides these services inaccordance with physician’s orders. Responsibilities: + Uses all available information sources to support care coordination activities-this may include portals, electronic medical records, claims data, plan information, utilization management information, and Milliman Care Guidelines (MCG). + Completes screenings, assessments and care plan in accordance with contractual requirements and Care Coordination policy and procedure. + Integrates information from claims review, notes, screenings, assessments and Medical Home teams in care plan. + Tracks patient progress regarding goal achievement on the care plan. + Updates documents based on Enrollee progress, changes in priorities, changes in health status or new information. + Supports care coordination referrals from multiple sources including Enrollee request, plan referrals, medical home referrals, grievances, data reports or changes in risk score. + Provides relevant education, counseling and support to assist member with the achievement of goals. + Collaborates with Nurse Care Coordinators on patients with multiple co-morbidities, frequent hospitalizations or inappropriate Emergency Department (ED) visits. Minimum Requirements: + Licensed in the State of Illinois as a Licensed Social Worker, Licensed Clinical Social Worker (LCSW), License Professional Counselor or Licensed Clinical Professional Counselor (LCPC) + Three (3) years of recent experience as a Social Worker + Prior care management work experience in a social service agency, physician group, hospital or Emergency Department setting + Valid Driver’s license and mandatory vehicle insurance or other means of transportation + Covid-19 Vaccine required Work Schedule: 8a-430p M-F In office: 2 days a week, Remote: 3 days a week Benefits At Amergis, we firmly believe that our employees are the heartbeat of our organization and we are happy to offer the following benefits: + Competitive pay & weekly paychecks + Health, dental, vision, and life insurance + 401(k) savings plan + Awards and recognition programs *Benefit eligibility is dependent on employment status.
