IDD Quality Outcome Coordinator Location: Middletown, NY, United StatesDate Posted: Jan 3, 2025 Share: share to e-mail Apply Now Save JobSaved Description Location: Middletown NY Hours: Mon-Fri 8a-4:30pm Rate $27/hr Other locations include New Windsor, NY Access: Supports for Living is seeking a dynamic and motivated Personal Outcome Measures (POM) Coordinator. The Coordinator organizes activities related to the completion and follow up with Personal Outcome Measures interviews. Job Duties: + Completes on average, 20 POM interviews with associated reports each month. + Ensures POM interview reports are entered into CQL and ACCESS Data Base. + Distributes all POM interview reports to the identified team members within one month of the completion of the POM interview. + Functions as a resource and support to all agency programs and employees in the delivery of CQL Personal Outcomes measures. + Provides written feedback and recommendations to Program Management on Individuals Personal Outcomes reviews. + Collaborates regularly with IDD team members to Supports to ensure Agency POM related goals are met. + Promotes Self-Advocacy and independence of individuals through CQL Personal Outcomes Measures education and support. + Coordinate and train others to become CQL certified interviewers. + Responsible to review and validate POMs interviews. Qualifications & Expectations + Bachelor’s Degree preferred. + Experience providing services to, adults, and older adults with intellectual and developmental disabilities, and psychiatric disabilities. + Proven success and experience in implementing CQL’s Basic Assurances and/or Personal Outcome Measures required. + Current CQL Certification status in the Adult Personal Outcome Measures and maintains CQL Interviewer Certification status by meeting all CQL requirements or obtain the above certification within the first year of employment. + Ability to perform effectively both independently and as a team member. + Proficient use of technology (Microsoft Office 365 apps/services – Outlook, Word, Excel, etc.). + Proficient in use of electronic health records (such as Evero). + Proficient use of virtual platforms such as Zoom and Microsoft Teams required. + Excellent organizational skills. + Ability to engage in travel to various sites to perform Personal Outcome Measures interviews + Demonstrated success in working with diverse teams. + Exceptional written and oral presentation skills to effectively communicate new ideas, concepts, and practices. + Alignment with CQL ACCESS and affiliates mission, vision, and values. + The ideal candidate will have the flexibility to work some non-traditional hours which would include evenings, and weekends. + Must have driver’s license and reliable transportation.
