The IMM RN Care Manager blends the mission of AHF with Care Management across the Managed Care, Department of Medicine and Pharmacy Divisions.  Utilizing strong nursing care management and clinical skills, the RN IMM Nurse focuses on a caring and knowledgeable approach. You’ll work with the IMM Team consisting of Pharmacists, Providers and RNs, so that patients receive the support they require and access to available resources.  This will include providing education and assistance so that eligibility for the program is understood. Working directly with the IMM Pharmacists, and IMM Providers to provide nursing interventions necessary to education, adherence and provide a successful pharmacy experience. You will complete Health Risk Assessments, develop Care Plans, and provide ongoing education to patients so that they understand their diagnoses and their medications.  You will provide support and assistance for patients who lack understanding or who better want to understand their health management needs. In this way you will support the patient’s ability to better understand their health status so that they can work to achieve their healthcare goals. This position is in-office, located at the Gardenia CA location of AHF.

 

By utilizing core principles of nursing, you will meet our patients where they are, identify strengths and opportunities for growth and assist them with realistic goal setting.  Advocating for your patient, you will bring the essence of the Ryan White Care Act - the safety net for the uninsured HIV/AIDS patients - to our most vulnerable population.  Completing a Health Risk Assessment, Care Plans, resources and support will directly contribute to AHF’s success in meeting the deliverables of the IMM program and URAC (Utilization Review Accreditation Commission) certification.
