YOUR CONTRIBUTION TO OUR SUCCESS!The IMM RN Care Manager contributes to AHF by working with the IMM Team, consisting of Clinical Pharmacists, Providers and RNs to educate patients on their medications and disease states, health screenings and sdoh screening to offer resources for unmet needs.
The IMM RN Care Manager enhances the reputation of AHF by providing Pharmacy patients with support and assistance in better understanding their health management needs.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.
The IMM RN Care Manager will work as a part of a close-knit team to assess our pharmacy patients and educate them on their disease processes and important health screenings.
Requirements: Florida Multistate RN License; ACRN encouraged within 2 years
Spanish preferred
