Independent Living Specialist - 85 Lexington Avenue
DUTIES & RESPONSIBILITIES: + Intake and assessment. + Individual and group counseling. + Concrete services, entitlements and advocacy. + Escort clients to various appointments. + Documentation, record keeping and progress notes. + Psychosocial, treatment planning and aftercare follow-up. + Attend staff meetings and trainings. + Housing placement + Provide emergency first aid/CPR assistance when needed. Related duties as assigned. HOURS: Full-time, 37.5 hours per week + Tuesday-Friday 10am-6:30pm; Saturday 1:30pm-10pm + Monday 1:30pm-10pm; Tuesday-Friday 9am-5:30pm QUALIFICATIONS: High School Diploma required. BA preferred, Bilingual (Spanish-English) preferred. Experience working with homeless mentally ill. Outreach, entitlement, substance abuse experience helpful. CASAC preferred. Proficiency in MS Office, including Word, Excel. PROGRAM DESCRIPTION: This 103-bed men's shelter in Brooklyn serves a population suffering from multiple issues including mental illness, substance abuse and serious medical problems. The program offers the full range of case management services as well as on-site psychiatric and medical care. It also incorporates a workforce development component for women who can benefit from this service. The Lexington Avenue Men's Residence is funded by the NYC Department of Homeless Services.
