Individual Program Coordinator
II. ESSENTIAL DUTIES AND RESPONSIBILITIES
+ Facilitates the development and implementation of each person's program plan assigned to caseload.
+ Assure the completion and annual update of assessments as needed for the development of the plan.
+ Train, coach and monitor staff or other people responsible for implementing of the program plan.
+ Write skill acquisition and assist with development of behavior reduction plans with input from other professional staff and those assigned direct responsibility for implementation of the program sites.
+ Assure the accurate and consistent implementation of the program plan at designated program sites.
+ Monitor the individual's progress on a monthly basis with interventions as necessary to assure that progress.
+ Assure accurate and consistent communication with all members of the individual's team, including parents, guardians, professionals and those responsible for the implementation of the program plan.
+ Coordinate services with others for people assigned to program sites.
+ Assure all service provision meets regulatory requirements including the necessary paperwork being completed in a timely manner.
+ Monitor services provision to assure the individual's rights are not infringed upon without written justification and Human rights committee approval.
+ Demonstrate active participation in the management team through consistent meeting attendance and presentation; provision of direct services to model appropriate interactions; support to direct care staff; and provision of performance feedback to staff and their direct care supervisor.
+ Assure that each individual has an accurate and manageable record. Together with house management and staff, if appropriate, develop and implement training to assure active treatment. This includes a series of specific training components such as initial training, procedural reliability, and data reliability.
+ Assist with implementation of departmental and agency Consumer Satisfaction protocols.
+ Ensure that data collection procedures to be completed by those responsible for implementation of the program plan components are accurate.
+ Ensure that all program material necessary for implementation of the program plan are available and in good repair.
+ Develop and maintain records of tracking for training specifics.
+ Serve as On-Call Administrator as required.
+ Complete assignments outlined in the Agency and Residential Quality Improvement Program.
+ Serve as a member of committee as required or requested.
+ Act as presenter of information to the Behavior Intervention Committee, Human Rights Committee or other committee as requested and pertinent to individual caseload.
+ Attend all required meetings as deemed necessary by supervisory staff.
+ This position requires a two week notice of voluntary resignation.
III. SUPERVISORY RESPONSIBILITIES
This position has no supervisory responsibilities.

Education and/or Experience- Bachelor's degree (B.A.) from a four-year college or university in a health service field and at least two years of experience working with people with developmental disabilities. Additional education may substitute for experience on a year for year basis.

The ability to work independently in multiple settings. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Language Skills- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write reports/correspondence and good communication and writing skills.
Mathematical Skills- Ability to add, subtract, multiply, and divide using whole numbers, common fractions and decimals. Ability to compute rate, ratio, and percentage and to draw and interpret bar or line graphs.
Computer Skills- Must be knowledgeable about Internet software, Microsoft Office components including Word, Office, Excel and PowerPoint. Becomes knowledgeable and trained in the use of Therap for plan writing, event reporting and ongoing communication.
Certifications, Licenses, Registrations- Must possess a valid NYS driver's license and an acceptable driving record. Successful completion of behavior intervention techniques, CPR/First Aid, and all other required training.

+ Physically able to bend/stoop, squat, push/pull, reach above/below shoulder level frequently.
+ Must be able to independently lift and/or transfer, push or pull up to 50 lbs. No lifting restrictions.
+ Regularly required to sit for long periods of time, walk and stand.
+ Ability to physically intervene using Agency-endorsed behavior interventions.
+ Must have adequate vision, speaking and hearing ability, with or without correction, to be constantly aware of, detect, and able to respond to each individual's needs when not in immediate vicinity.
+ Talk or hear both in person and by telephone.
+ Specific vision abilities required include close vision, color vision, and ability to adjust focus.
+ Sit and use hands to finger, handle, or feel objects, tools, or controls.
+ Ability to ambulate; reach with hands and arms.
+ Ability to operate a computer and software programs.

VI. WORK ENVIRONMENT

+ Located in a busy, open area meeting people on a regular basis.
+ Works with household cleaning solutions.
+ Moderate noise; 75-90% of work is performed indoors, 10-25% outdoors.
+ Works with wheelchairs, electric/hydraulic/manual lift/ramp equipment, lifts and hook-ups, oxygen tanks, mechanical/electrical beds, household appliances, cleaning equipment, whirlpool machines, mist machines, suction machines, augmentative communication devices, yard equipment and office equipment.
+ Wear proper Personal Protective Equipment at all times.
+ Works with office equipment.
+ Must clean up human waste and other body fluids as required.
+ Responsibilities occasionally may require an adjusted work schedule including overtime.
