Position Snapshot: Perform NY Connects functions so that more individuals will have access to long-term care supports and services (LTSS) and live independently in their homes and communities.
Essential Functions:
+ Strong commitment to and belief in the independent living philosophy and in community integration for all people with disabilities.
+ Receive referrals from the No Wrong Door (NWD) and other community-based agencies.
+ Provide information and referral, person-centered planning options counseling, benefits advisement, and Medicaid application assistance for individuals and families who desire to live independently.
+ Provide unbiased information regarding available home- and community-based services, participate in discharge planning, and coordinate with other relevant community-based agencies.
+ Utilize the NWD assessment tool to ascertain the likelihood of eligibility for public benefits and LTSS. Assist in completing applications and enrollment, as appropriate, or make linkages to other community-based agencies.
+ Collect and maintain all required statistical and other data and prepare reports in a timely manner.
Candidate Requirements need to include:
+ Bachelor’s in human services or related field, preferred but not essential.
+ Personal experience with a disability and/or a senior citizen 60 years of age or older is strongly preferred.
+ Experience working in the long-term care system is required.
+ Advanced knowledge of OPWDD and OMH supports and services, as well as other community-based organizations
+ Knowledge of various waivers (TBI, NHTD, OPWDD) is preferred.
+ Excellent communication, writing, and computer skills.
