Uses patient interviews and counseling, reviews the medical record and medication profile, consults with physicians, teaching, staff education, medication use evaluation, adverse reaction monitoring and resolves medication-related problems. Interventions target high risk population through the provision of medication reconciliation and comprehensive medication use management in focused disease states.
