Use the nursing process to plan and provide patient/family care. This includes assessing and evaluating needs and implementing effective care plans which are kept up to date, evaluated and documented..Coordinate patient care activities and maintain communication with other disciplines including nursing agency, pharmacist and physician.Teach the patient/family about the patient’s care needs and disease prevention.Follow appropriate infection control measures.Company DescriptionThe University of Maryland Medical System (UMMS) is an academic private health system, focused on delivering compassionate, high quality care and putting discovery and innovation into practice at the bedside. QualificationsEducation and Experience
Minimum of one year home health experienceRegistered Nurse, currently licensed by the state of MarylandCRNI credential preferredKnowledge, Skills and Abilities
