Summary:
UNC Medical Center Department of Pharmacy has an opening for a highly motivated pharmacist seeking to join our inpatient clinical oncology team in Chapel Hill, NC. The Inpatient Hematology Oncology Pharmacist Specialist plays an integral role in the management of patients with hematologic malignancies and solid tumors. This individual will gain extensive experience in the therapeutic management of hematology and oncology patients through rotation across several specialized services, including medical oncology, hematology, and gynecology oncology. The inpatient specialist collaborates with physicians, APPs, nurses, and the operational inpatient pharmacy team to provide comprehensive clinical care to a diverse patient population within the hematology oncology space. The position offers the opportunity to develop preceptorship and research skills through pharmacy student and PGY1/PGY2 resident precepting and mentorship.
Activities include multidisciplinary rounding, preparation of appropriate orders, responding to drug information requests, medication profile review, order verification, nursing support, patient counseling, and transitions of care.
This individual will report to the Clinical Manager of the Hematology/Oncology Service Line and work closely with the Hematology/Oncology Clinical and Central Specialists as well as the Specialty Pharmacists within the UNC Carolina Care Pharmacy Care Network.
RESPONSIBILITIES:
The primary responsibilities of the Inpatient Hematology Oncology Pharmacist Specialist include providing comprehensive pharmacist care to patients seen on the Inpatient Hematology and Oncology Services, collaborating closely with Outpatient Clinical Pharmacist Practitioners to ensure continuity of care between acute and ambulatory services, and leading quality improvement and outcomes initiatives.
**Clinical Service Responsibilities**
+ Participate in daily multidisciplinary patient care rounds
+ Respond to drug information questions from the medical staff, nursing staff, or pharmacy colleagues
+ Function as a limited oncology prescribing practitioner (LOPP) to collaboratively manage chemotherapy treatment plans
+ Conduct medication profile review and order verification
+ Evaluate, monitor, and redesign medication regimens to ensure positive patient outcomes
+ Provide patient education
+ Complete Therapeutic Drug Monitoring
+ Collaborate with the ambulatory care pharmacists and the chemotherapy infusion pharmacy team on policies, procedures, and guidelines associated with this patient population
**Service Responsibilities**
+ Serve as a standing committee member on a select group of committees within UNC Hospitals
+ Contribute to ongoing quality assurance and outcomes initiatives for the Department of Pharmacy, the Hematology/Oncology Division, and UNC Hospitals
**Educational Responsibilities**
+ Educate students from the UNC Eshelman School of Pharmacy, PGY1 and PGY2 residents from UNC Hospitals on their experiential rotations
+ Appointment at UNC Eshelman School of Pharmacy is available with commensurate experience.

**Education Requirements:**
● PharmD or BS Pharmacy Degree
**Licensure/Certification Requirements:**
● Active pharmacist licensure with the NC Board of Pharmacy or obtainable before initiation of employment. Recent graduates of ACPE-accredited colleges/schools of pharmacy may fill this role while pending initial pharmacist licensure, but are subject to limitations mandated by the NC Board of Pharmacy and must obtain licensure within 6 months of orientation or sooner if required for operations.
**Knowledge/Skills/and Abilities Requirements:**
+ Extensive clinical skills and pharmacotherapy knowledge base in Hematology/Oncology
+ Strong oral and written communication, organizational and prioritization skills
+ Ability to work in team environment
+ Ability to generate meaningful contributions related to oncology pharmacy practice and advancement of pharmacy practice.
+ Ability to design and implement projects, collect and interpret data accurately, and prepare project analyses/summaries/manuscripts
**Preferred:**
+ Completion of a Pharmacy Practice (PGY1) Residency or equivalent experience
+ Completion of an Oncology (PGY2) Pharmacy Residency or equivalent experience
+ Board Certification in Oncology Pharmacy
