Our flagship transitional work program– Ready, Willing & Able– integrates paid work, occupational training, and holistic supportive services to facilitate an individual’s return to mainstream society.
We are presently seeing an Intake Case Manager to join our organization at our Porter location.
RESPONSIBILITIES:
Meet with every new Client to introduce program structure, guidelines, and expectations Orient Client to new surroundings, provide initial bedding and toiletries Complete detailed interview and paper intake with Client Enter all intake information into two databases (CARES & TDF Database) Construct initial Independent Living Plan (ILP) with every new Client Assist with House Management duties whenever possible, including digital bed management, urine collection for toxicology, building rounds, etc.QUALIFICATIONS:
High School Diploma or Equivalent Experience with criminal justice or homeless population, or ability and desire to learn and adjust quickly to the population Excellent Client contact skills, including a dedication to treating all Clients with dignity and respect Computer Proficiency (must master two databases, and position relies heavily on email contact) Must be an independent worker, and able to navigate varying situations without onsite Supervisory supportInterested?
