Job Title: Intake Coordinator

Work Location: Brockton, Ma.

 Job Summary:

We're in search of a intake coordinator to work on a full time basis at with our Behavioral Health & Recovery Services Division. The intake coordinator is accountable to the Practice Manager, who is responsible for providing supervision and training. The intake coordinator is one who actively promotes the philosophy of the Gándara Center and maintains a high level of commitment to the program.

Duties and Responsibilities

Provide courteous and timely customer service Initiate and complete all administrative paperwork included in the Person Served Intake Packet Ensure that intake paperwork meets quality assurance standards [as determined by Senior Director or designee] and is entered into the electronic medical record Respond to all intake calls and voicemails within two business days Orient persons served to service availabilities, rights and responsibilities Ensure documentation is entered into any and all database and relevant systems Communicate in a timely manner with Practice Manager when additional resources or support is needed Assist with phone calls and translations as needed Travel, as needed

Administrative Responsibilities/Record Keeping

Adhere to program and agency policies and procedures Ensure that required documentation is completed professionally and accurately and filed in the electronic health record in a timely manner Attend scheduled supervisory meetings Complete time sheets and submits them accurately and in a timely manner Maintain job related record keeping accurately and submit reports in a timely manner

Minimum Qualifications:

High school degree or equivalent; post-secondary education an asset  At least 2 years of clerical experience  Bilingual skills in Spanish-English required  Microsoft Office proficient (e.g. Excel, Word, Power Point)  Flexibility to work shifts, including evenings, as required  Ability to organize, prioritize and work under extreme work pressure, heavy workloads and deadlines  Positive attitude and work ethic  Excellent verbal and communication skills  Must be team oriented
