Intake Coordinator

Castle Partial Care Program

Population: Children & Adolescents

Location: 1000 Atlantic Avenue Camden NJ 08104 (Virtua facility)

One day remote/four days onsite

$24-$27 per hour

 

Responsibilities

 

Provide individual and family treatment.Carry a caseload not to exceed 4.Facilitate group therapy as needed.Acts as a liaison between referral sources, the patient and family and the program. Follow-up with referral sources, the patient and family and program.Complete discharge follow-up phone calls.Assist therapists in case management duties such as scheduling patient appointments, requesting documentation, etc.Assist in tracking referral and discharge statistics.Completes Needs Assessments for Perform Care for all intakes.

 

Qualifications Bachelor's degree in Social Work or related field.Experience  with medical front desk scheduling and great customer services. Excellent interpersonal skills.Familiarity with community mental health resourcesExcellent organizational and time management skills.Ability to accept supervision and guidanceDemonstrated ability to work independently.
