TheIntake Coordinatoris primarily responsible for accepting and processing referrals for the Juvenile Justice for Youth and Family Department (JYF programs: NSP, NSD, LSP) and up to two additional departments as needed. The position is on-call and provides screenings for intakes to decide eligibility as well as if the referral can be accepted. As a member of the organizations centralize referral hub, the coordinator will also be responsible for providing resources to both internal and external referral sources.
Responsibilities
+ Receive, process, and distribute new intake referrals to appropriate JYF programs. (NSP, NSD, LSP)
+ Responsible for collecting data and following up with intake referral sources.
+ Develop strong relationships and partnerships with external referral sources.
+ Maintain records of intake referral inquiries and admissions.
+ Responsible for processing new intake admissions to the appropriate programs.
+ Responsible for entering new intake referrals through knowledge management systems (CONNECTIONS, MYEVOLV and LEWIS)
+ Responsible for assisting A/R team with managing on-call intake hotline and incoming resource referrals, includes non-business hours.
+ Cross train in all intake departments to assist and support team as needed.
+ Maintain formal tracking system to measure agency service referrals performance as it relates to marketing and relationships with referral resources.
+ Enters referral and agency intakes information in knowledge management systems (MyEvolv, CONNECTIONS, LEWIS, etc.)
+ Participate in community partnership meetings and workshops.
+ Assist in events to represent the Access and Referral department.
Qualifications
+ BA preferred.
+ High School Diploma/ GED required with a minimum of 1 - 2 years experience in access and referral intake.
+ Strong affinity for customer service.
+ Excellent communication skills both verbal and written.
+ Knowledgeable in Knowledge management systems.
+ Prior experience with admissions or Public Relations.
+ Spanish speaking preferred.
Work Environment:
This role requiresthree in-officedays andtwo remoteday per week, with flexibility for after-hours, weekend, and holiday support as needed. The position involves collaboration with multiple teams, fieldwork, and travel to different facilities to assess while maintaining a safe and proper environment for the children.
Position Type/Expected Hours of Work:
This is a full-time position with annual salary $52,000.00. The position operates Monday through Friday in our Harlem New York office at 1751Park Avenue NY, NY. Work hours options are 9:00 am - 5:00 pm.
