Communicating with patients, families, and medical professionals
Ensuring all required documentation is completed accurately and timely
Collaborating with internal teams to ensure smooth transitions for patients
Managing patient records and ensuring compliance with agency policies and state regulations
Proven experience as an Intake Coordinator in a home health agency
Excellent communication and organizational skills
Familiarity with medical terminology and home health regulations
Ability to work in a fast-paced environment and handle multiple tasks simultaneously
