Intake/Scheduling Assistant

Overview
Accurately enters all appropriate patient information into EMR
Schedules all PT and SN admissions and evaluations
Coordinates scheduling of all home health visits
Monitors compliance with Therapy Functional Assessments and supervisory visits
Monitors staff productivity and communicates with Clinical Managers
Completes any required audits
Triages Intake phone calls and referrals
Assists the Intake Department as needed
Communicates request for authorizations with appropriate personnel
QualificationsHigh School graduate required. Associates Degree preferred. 2-3 years of Home Health experience preferred. Excellent computer skills required.
