Intake Specialist
GENERAL FUNCTION: This position is responsible for processing referrals and facilitating intakes and for New Directions Transitional Housing Program. Assists with matters related to census, program growth, intake coordination, outreach, and program functioning. + Coordinate and conduct comprehensive screening and intake interviews with potential participants to assess their needs and program eligibility and in accordance to documentation policies. + Assure a transitional housing census in New Directions per agency criteria. + Assist with assigning new clients to Advocates. + Assist in roommate matching and roommate mediation meetings. + Work with the development team to identify marketing needs and implement marketing strategies to promote the transitional housing program. + Present and market program to surrounding communities and community partners. + Interface with Complex Managers to ensure smooth operations with program intakes. + Review and approve intake applications based on program criteria + Work closely with apartment complexes, placement agencies, authorized representatives, county social workers, and community supports to coordinate intakes and gather necessary documentation/information. + Assists Program Management with apartment acquisitions and as liaison between program participants and apartment complexes. + Ensure new apartments are move-in ready and equipped with all necessary utilities, furnishings, and resources for participants. + Assists with inventory management without waste and within program budget according to agency policies and procedures. + Assists Program Management with required outcome monitoring. + Assists with required agency and County database entry and reports. + Assists Program Management with linkage to outside educational, financial, health, mental health, and employment resources. + Initiates connections with outside educational, financial support, health, community and employment resources to facilitate delivery of services in partnership with program staff. + Assists with apartment moves and cleaning of apartments as needed. + Assists with ensuring compliance with county contract requirements, Community Care Licensing regulations, Joint Commission accreditation standards, and agency policies and procedures. + Assures that volunteer resources are used effectively within the program. + Other duties as assigned. QUALIFICATIONS: + Must be able to be associated with Casa de Amparo through Community Care Licensing (CCL), which typically includes clearance by FBI, DOJ and CACI + Bachelor’s degree in social work, psychology, human services, or related field required + Minimum of two years’ experience in a public or private social services setting preferred + Experience in social services, Transitional Age Youth (TAY), or foster youth preferred + Strong understanding of TAY development, the effects of trauma, California Title 22 regulations and San Diego County private/public social service agencies. + Well-organized, strong telephone and interpersonal communication and interviewing skills, detail and big picture oriented. + Ability to comply with all employment requirements including, but not limited to drive agency vehicles with clear DMV report, reliable transportation with auto insurance. + Must provide proof of immunization against influenza, pertussis, and measles. + Must have the physical ability to participate in activities and perform related physical skills, including, but not limited to, crouching, lifting 50 pounds, crawling on hands and knees, and walking up and down stairs. + Must have the physical ability to participate in activities and perform related physical skills, including, but not limited to, crouching, lifting 50 pounds, crawling on hands and knees, and walking up and down stairs. + CPR/1st Aid (including pediatric) certificate required. + Must pass a pre-employment drug screen, health exam and TB test. + Must be at least 21 years of age EFFECT ON END RESULT: This position assures the culturally responsive trauma-informed screening and intakes for participants in the New Directions Transitional Housing program according to census goals. The Specialist will ensure accurate, timely, organized completion of documentation according to governing bodies and agency policies and procedures. This includes the professional outreach and development of relationships within the community to support program services and census. This job description is not intended to be all-inclusive.
