Job Description
We are seeking a dedicated Integrated Case Manager responsible for case management and discharge planning of patients from the hospital to their home or another medical facility. This role involves creating and implementing patients' discharge plans from an office setting using electronic medical records (EMR) systems, predominantly EPIC. The case load typically follows a 1:25 ratio, ensuring manageable workloads for staff.
ResponsibilitiesConduct comprehensive assessments of patients and their family/caregiver’s biomedical, psychological, social, and functional needs.Develop personalized, patient-centered care plans aimed at optimizing the patient's care experience.Engage patients and their families as part of the care team through advocacy, ongoing communication, health education, identification of resources, and service facilitation.Utilize professional judgment, critical thinking, motivational interviewing, and self-management techniques to assist patients in overcoming barriers to goal achievement.Provide counseling and interventions related to treatment decisions and end-of-life issues, including Advanced Care Planning.Coordinate as necessary to ensure patients seamlessly and safely transition between care settings.Advocate for appropriate delivery of services within the patient’s health plan benefit structure.Collaborate with appropriate members of the patient’s treatment/care team to co-manage patients with complex medical and social needs and facilitate interdependent collaborative care conferences.Continually evaluate the patient's response to the care/treatment plan, making modifications when necessary.Plan and participate in process improvement activities designed to reduce risk, inclusive of data collection, analysis, and follow-up intervention activities.Facilitate interventions in cases involving child abuse and neglect, domestic violence, elder abuse, institutional abuse, and sexual abuse.Essential SkillsRegistered Nurse (RN) or Licensed Social Worker (LSW) with a valid, unrestricted State of Michigan license.Bachelor's or Associate's Degree.At least three years of inpatient clinical case management experience and discharge planning experience in an acute care setting.Basic Life Support (BLS) certification.Additional Skills & QualificationsExperience with multiple hospital units, such as Cardio, Neuro, Neuro ICU, and ICU.Experience with electronic medical records systems, preferably EPIC.Strong communication and advocacy skills.Proficiency in motivational interviewing and self-management techniques.Work Environment
This role involves working at a computer/desk most of the day within the assigned floor/unit. The office unit consists of approximately 20-25 case managers. The work environment is fast-paced, and the case manager is expected to complete their case load to ensure the workload is balanced among the team. The position includes rotating weekend shifts every 3rd-6th weekend, with advance scheduling and compensatory time off during the week.
