Job Description
We are seeking a dedicated Integrated Case Manager to oversee the case management and discharge planning of patients transitioning from the hospital to their home or another medical facility. You will primarily work on a specific unit, but may assist in other units as needed. This role involves creating and implementing patient discharge plans from an office setting using a computer. The electronic medical records system used is EPIC, although prior experience with EPIC is not mandatory.
ResponsibilitiesConduct comprehensive assessments of patients and their family/caregiver’s biomedical, psychological, social, and functional needs to gauge the potential impact on recovery.Develop personalized patient-centered care plans aimed at optimizing the patient’s care experience.Engage patients and their families as part of the care team through advocacy, ongoing communication, health education, identification of resources, and service facilitation.Utilize professional judgment, critical thinking, motivational interviewing, and self-management techniques to assist patients in overcoming barriers to goal achievement.Provide counseling and interventions related to treatment decisions and end-of-life issues, including Advanced Care Planning.Ensure patients seamlessly and safely transition between care settings through necessary coordination.Advocate for appropriate delivery of services within the patient’s health plan benefit structure.Collaborate with appropriate members of the patient’s treatment/care team to co-manage patients with complex medical and social needs. Facilitate interdependent collaborative care conferences.Continually evaluate the patient’s response to the care/treatment plan, making modifications when necessary.Plan and participate in process improvement activities designed to reduce risk, including data collection, analysis, and follow-up intervention activities.Facilitate interventions in cases involving child abuse and neglect, domestic violence, elder abuse, institutional abuse, and sexual abuse.Essential SkillsRegistered Nurse (RN) or Licensed Social Worker (LSW) with a valid, unrestricted State of Michigan license.Bachelor's or Associate's Degree.At least three years of Inpatient Clinical Case Management experience and Discharge Planning experience in an acute care setting.Basic Life Support (BLS) certification.Experience with EPIC EMR.Additional Skills & QualificationsExtensive discharge planning experience within multiple units of the hospital.Work Environment
You will work at a computer/desk most of the day on the floor/unit assigned. The office unit consists of about 20-25 case managers. This is a fast-paced environment where you will be counted on to complete your case load, ensuring that additional work is not placed on the rest of the team. You may work on any of the medical floors, including Cardio, ICU, etc. The working hours are from 8:30 AM to 5:00 PM, with rotating weekends every 3rd to 6th week. These are 13-week assignments with potential 13-week extensions based on performance metrics, with opportunities for direct hire and long-term consulting.
Pay and Benefits
The pay range for this position is $47.00 - $53.00/hr.
