The Integrated Healthcare Liaison is responsible for providing clinical, case management, and consultation services to children and adolescents. Also provides counseling, therapy, crisis intervention, assessment, psychoeducation, and referrals to children and families served. Works closely with child psychiatrists in providing ongoing assessment, education and support to youth receiving psychiatric services. Develops and maintains working relationships with youth and their families and/or care givers, NRVCS staff, other unit staff, and agencies/organizations also involved in the youth's care and participates in service coordination to ensure continuity in the child's care. In addition, works closely with Pediatric Offices to ensure timely exchange of pertinent medical/psychological information as it relates to crisis clients, including change of medications, and current functioning of clients identified by either Pediatrician or Psychiatrist as being at risk emotionally. The position will monitor the transition to and from Primary Care to Psychiatric providers as needed, in addition to facilitating communication between NRVCS programs and Primary Care Offices, serving liaison for the enhancement of integrated care.
Starting Salary: $51,267-Depending on Experience
Education/Experience Required:
Graduation from an accredited college or university with a Master's degree in education, social work, psychology or human service related field. Licensed or licensed-eligible is required. At least five years of experience in a setting working with individuals with emotional, mental health, intellectual disabilities and/or substance use disorders is required. The ideal candidate will have previous experience in Primary Care.
