POSITION SUMMARY:
The IDD Services Coordinator will assist in program development, marketing, staff training and service monitoring for the clinic’s Intellectual and Developmental Disabilities Program. This position will take the lead role in coordinating the activities of the IDD team as well as collecting and reporting all necessary data to the supervisor. This position will also maintain a caseload with children in our 0-17 supports coordination services.
EDUCATION AND EXPERIENCE REQUIRED :
+ Bachelor’s degree in social work, psychology or other related human service field required.
+ QIDP credential or credential eligible and maintenance of that credential.
+ Must have reliable mode of transportation and must possess a current valid State of Michigan driver’s license and automobile insurance that is insurable.
+ At least two years direct clinical experience in providing 0-17 home-based services required. This includes IDD services assessment, IDD services intervention, local resource knowledge and treatment planning.
DUTIES AND RESPONSIBILITIES:
+ Work as part of an interdisciplinary team to provide assessment, treatment and support to youth and families in the 0-17 IDD programming.
+ Provide anticipatory guidance to other professionals working with the 0-17 IDD population. This includes training, technical assistance, and monitoring and coordination of assessments and interventions for children.
+ Provide treatment planning and support services with measurable objectives in the home and community settings. Provide support and crisis intervention with families involved with the program.
+ Assist in identification and reduction of cross-system barriers.
+ Identify and work closely with supervisor and staff on program development and client needs including but not limited to outcome measures and training needs.
+ Work cooperatively as a member of a team with program staff, supervisors, and community referral sources.
+ Participate in marketing and outreach activities, develop, and maintain referral relationships with daycares/schools, medical agencies, and other referral sources.
+ Maintain client files. Submit timely and accurate documentation of services, billing data and required paperwork according to Starfish policies and procedures.
+ Monitor program referrals and assignment of caseloads to the 0-17 IDD program. Responsible for working with team members, supervisor, children, families, and community contacts in a manner that is conducive to the philosophy and mission of the program and agency.
+ Participate in individual and group supervision.
+ Participate in on-going personal and professional development including in-service training, peer review, external workshops, and seminars.
+ Accept and emulate the mission of SFS.
+ Assist in finding and directing clients in case of an emergency such as a fire or tornado.
+ Cooperate in any investigation related to personnel, licensing, accreditation, or other circumstances.
+ Attend all agency and departmental meetings and training as required.
+ Learn and be able to support others in following Starfish Family Services and Behavioral Health policies and procedures.
+ Perform other related duties as assigned by supervisor or other management.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
+ Knowledge of typical childhood development, as well as socio-emotional, physical, and cognitive disabilities.
+ Willingness to provide group training/education to other professionals.
+ Ability to manage multiple priorities, function independently off site and demonstrate good follow-through and problem-solving skills.
+ Ability to demonstrate commitment, caring and respect for children and adults from diverse backgrounds who have multiple needs or concerns.
+ Ability to work cooperatively and responsibly as a member of a team with colleagues, supervisors, agency staff and community referral contacts.
+ Ability to develop treatment plans and coordinate needed services to fulfill plans.
+ Ability to role model appropriate parent management techniques to parents and staff.
+ Ability to make critical decisions independent of immediate supervision.
+ Knowledge of community resources and how to link and refer.
+ Competence in the delivery of crisis intervention services.
+ Must be computer literate- email, electronic medical records, and Microsoft office at minimum.
+ Ability to effectively, appropriately, and accurately communicate both orally and in writing.
+ Ability to meet client service hours expectation as assigned by program supervisor.
+ Ability and willingness to abide by all Starfish Family Services confidentiality policies.
+ Ability and desire for personal and professional growth, and skill development.
+ Ability to give and receive constructive feedback.
+ Must have a commitment to the welfare of the children and families the program serves and to the delivery of quality services.
+ Ability to work flexible hours as needed (i.e., some evening or weekend work can be expected).

LICENSING AND OTHER REQUIREMENTS:
+ Valid driver’s license and / or reliable transportation.
