Intensive Family Based Services Coordinator - Children's Services - White River Junction

Position Purpose:

A grant-funded position through the Department of Children and Families, the IFBS Program Coordinator works with the IFBS clinician and IFBS Case Manager to provide family screening and assessment, family plan, brief and intensive therapeutic and behavioral interventions that are often based in the family home, parenting education and support services and linkages to other services for families at risk, with the goal of building the family’s internal resources to prevent abuse or neglect. May have the task of helping parents accessing treatment for mental health and/or substance abuse and addiction treatment needs. Will be responsible for supervision of IFBS Case Manager, coordination of all IFBS services and monthly reporting and billing.

 

Principal Responsibilities and Duties:

 Assess family social, emotional and behavioral functioning and intervention needs in collaboration with IFBS Clinician, family and DCF in order to develop plan for treatment and service provision. Work with families to improve styles of communication and parenting. Provide education for families on the developmental needs of children. Works in partnership with the IFBS Clinician and IFBS Case Manager and DCF District Director/Open Family Cases Supervisor to coordinate services and supports to families including mental health and substance abuse treatment. Collects and compiles data on individual levels of functioning. Collaborates with other organizations, service providers, and individuals in the agency and community relative to family’s needs and goals. Maintains accurate client case records in a timely fashion. May serve as the family case manager with schools, residential facilities or social service agencies. Consults with staff and families, and participates in planning special programs and training/educational activities. Provides documentation as needed in legal proceedings and appears in court as required. Responsible for overseeing the administrative and documentation requirements of the IFBS program, such as Monthly, Quarterly and Annual billing and notes.

Relationships:

Supervised by Area Manager/Clinical Supervisor Coordinates with the client/family’s treatment team, including DCF social worker. Works closely with wide range of people within and outside of the agency to coordinate services to the family.

 

Qualification Statements

EDUCATION and/or EXPERIENCE:

A Bachelor’s degree in a relevant field with demonstrated excellent community supports and family case management skills preferred. At least four years of experience preferred, working with families at risk. Knowledge of family systems, behavior management, substance abuse and other areas.

 

LANGUAGE SKILLS:

Strong written and verbal communication skills. Ability to listen effectively. Ability to read, analyze and interpret documents such as: psychological evaluations, clinical reports, safety plans and rules, and common scientific and technical journals. Ability to respond effectively to common inquiries and complaints from consumers, families and others involved with consumers, and regulatory agencies.   Ability to effectively present information to senior managers. Ability to keep clear, up-to-date consumer files organized to meet regulatory standards.   Ability to write routine reports and correspondence.

 

MATHEMATICAL SKILLS:

Ability to understand and interpret test scores and other mathematical information presented in reports on consumers. Ability to understand the mathematics involved in collecting data for reviews, audits, and continuous improvement efforts.

 

REASONING ABILITY:

Ability to define problems, collect data, establish facts, and draw valid conclusions.   Ability to interpret an extensive variety of technical instructions presented in mathematical or diagram form and deal with several variables simultaneously.

OTHER SKILLS AND ABILITIES:

Ability to work flexible hours. Knowledge of appropriate ways to contact Emergency Services Team. Knowledge of appropriate physical restraint techniques may be required. Safe, legal transportation.

The employee may occasionally lift and/or move up to 25 pounds.

Specific vision abilities required by this job include close vision, and the ability to adjust focus to perform such tasks as reading reports or driving.
