FirstChoice Leaders fill interim leadership assignments across our nationwide healthcare system._** **_Start: ASAP_** **_Des Moines, Iowa_** **_13 weeks_** **_$3400.00 gross weekly_** **_Plus if 60+ miles from facility, $1300.00 Tax Free weekly Stipend or Trinity Health Pays Travel and Lodging._** **POSITION PURPOSE:** Provides leadership, accountability and responsibility for the entire scope of Care Coordination for MercyOne Medical Center and MercyOne West Des Moines. Operational responsibilities include budgeting, strategic planning, patient care management and integration and achieve organizational goals. Acts as a role model and change agent by initiating and accepting changes for improved patient care and cost reductions. Maintains active communication and collaborates with senior leadership, physicians, leaders, and staff to meet specialty, departmental and organizational goals. **MINIMUM QUALIFICATIONS:** + Current Iowa License Registered Nurse or Social Worker + Master’s degree in healthcare related field required. + Five years previous experience in large, complex care coordination at a director level or equivalent. + 3 years previous acute clinical care coordination experience, preferred. + Proof of completion of Iowa’s Mandatory reporter abuse training specific to population served within three (3) months of hire. **Our Commitment to Diversity and Inclusion** Trinity Health is one of the largest not-for-profit, Catholic healthcare systems in the nation.
