The Community Resource Navigator will play a vital role within the police department’s Co-Response Jail Diversion Program. This position is designed to provide essential support to individuals facing mental health, substance use, or social service challenges. The Community Resource Navigator will work collaboratively with law enforcement, mental health professionals, and community resources to ensure individuals receive appropriate follow-up services, aiming to reduce recidivism and improve overall community health outcomes.

Minimum Education Required Bachelor's Degree Additional Shift Details Part time 25 hours a week Responsibilities Conduct comprehensive assessments of individuals in crisis to identify their needs and connect them with appropriate resources and services.Develop individualized care plans and facilitate connections to mental health, substance use treatment, housing, and social services.Work closely with police officers, mental health professionals, and community organizations.Ride with police officers in cruisers for knock and talks to provide in person introduction to services and supports.*Monitor progress and outcomes of individuals in the program, ensuring they access ongoing services and support.Maintain accurate records of assessments, care plans, and follow-up services provided, ensuring compliance with Advocates policies and regulations.Engage with local organizations and stakeholders to promote awareness of the program and foster collaboration for improved resource availabilityKeep statistics on direct hours, frequency of police interactions, and demographics of each person served.

Qualifications Bachelor’s degree in social work, psychology, public health, or a related field..Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populationsExperience in mental health, substance use, or crisis intervention settingsKnowledge of community resources and services relevant to individuals in crisis.Must adhere to professional ethics.Basic computer knowledge.Ability to manage multiple tasks simultaneously.Commitment to Advocates values and mission.Ability to develop and maintain satisfactory working relationships with employees and providers.Ability to communicate effectively verbally and in writing.Ability to read and speak English.Sensitivity to the needs of the population.High energy level, superior interpersonal skills and ability to function in a team atmosphereMust have valid driver’s license and access to automoileMust be able to perform each essential duty satisfactorily.
