ACLD’s Job Coach provides on-site job coaching and advocacy for people we support in the intensive and follow-along phases of Supported Employment.

 

In this position, you will:

 

Provide on-site job coaching for designated caseloadComplete and ensure accurate and timely documentation of SAP plans, case notes, monthlies, DDPs and other required documentation.Conduct observational assessments on site in order to assist the person supported in learning the tasks of the jobTravel train on the public transportation system and/or roadwaysMaintain case files for each person supported.        Prepare all documentation for ACCES and OPWDD on a monthly basisAct as a liaison between the person supported, family and employerAttend all staff meetings, Life Plans, conferences and annual training requirementsProvide overall assistance as assigned

 

We require:

High School Diploma or equivalent; Associate’s Degree in Psychology, Sociology, Education or related field preferred.Minimum of one year experience providing vocational or pre-vocational services to supported persons preferredComplete (within first year of employment) twenty-four (24) hours of OPWDD’s Innovations in Employment Supports training.Clean, valid New York State driver’s license
