Job Coach Manger

Hourly Wage: $19.50- $22.00
Chrysalis is a company that provides a variety of support services for at-risk adults and/or juveniles with intellectual disabilities. These individuals may also have a variety of medical issues. This position requires the ability to work in stress-filled and safety-sensitive environments, make crucial decisions in regards to the care and treatment of at-risk individuals, and possess the ability to communicate those decisions effectively to the individual's team. Under the general supervision of the Employment Specialist, the Job Coach will be responsible to facilitate Vocational Rehabilitation supported and customized employment services. These services include training the individuals on essential job duties, establishing relationships with the individual’s employer and coworkers, supervising the individuals in their job, and all other functions independently within the constraints of Chrysalis policies and procedure to assure individuals receive the highest quality of services possible.
Essential Duties and Responsibilities:
Communicate effectively with individuals.Punctuality and regular/consistent attendance at the worksite are required and the employee must be able to adhere to their workplace schedule to ensure proper oversight of individuals. Stay awake, responsive, and communicate effectively with individuals.Conduct interviews.Supervise all employment team members and conduct plan of action plans.Build relationships with employers.Teach individuals daily living skills.Manage employment calendars.Attending Team meeting for clients.Prepare ACRE correlation agenda.Attend VR Meetings.Daily monitoring of employer needs.Plan appropriate activities for the individuals.Create opportunities for people to increase quality of life.Complete the USOR 93 form for each individual monthly.Update Employment Support Plans.Submit timesheets on time.Track hours.Coordinate with Job Coaches and Residential managers.Audit and upload VR summaries into fusion correctly and on time.Request Job Coaching Authorizations.Complete lump sum authorization forms to justify billing.Support the individual’s choices.Provide a clean, healthy, and safe environment.Implement Person Centered Plans.Supervise individuals according to the Supervision Guidelines.Follow Behavioral Plans.Covering Job Coach shifts.Assist and supervise individuals with taking the appropriate medication.Complete necessary administrative tasks within established time frames.Complete and document all documentation required by Chrysalis and the State.Maintain communication with the Residential manager regarding the individual’s needsRequirements
Minimum of least 21 years old. Pass and maintain a passable criminal background check and LEIE per state regulations.Be eligible to drive according to Chrysalis policy.Should possess the ability to speak, read and write effectively in English.Training and Certifications:
Certified through Association of Community Rehabilitation Educator (ACRE) Employment Services Basic Level.Complete all initial and on-going training requirements within established time frames.Complete annual CPR, First Aid, and OSAH training (All Employees).Obtain and maintain SOAR certification (All Employees).Essential Knowledge, Skills, and Abilities:
Understand policies and follow the Provider Code of Conduct/ Abuse, Neglect and Exploitation Policy according to state standards. Excellent public relations skills working with the individuals’ family. Ability to communicate effectively with medical, professional, and state employees.Demonstrate good judgment and ability to handle crisis situations.Have excellent interpersonal skills working with the individual and co-workers.
