Joint Center Coordinator, Day Shift, Med Surg Ortho

As a Joint Center Coordinator, you will:
• Conduct the pre-operative Joint Workshop Classes for patients scheduled for joint replacement procedures.
• Coordinate with care team of patient during the acute hospitalization to enhance patient satisfaction and ensure quality care including rounding during hospitalization and post discharge follow up phone calls.
• Collaborate with surgeons/physicians, nursing, therapies, operating room staff, marketing, food service, pharmacy, outreach coordinators, clinic staff, and all members of the healthcare team through team conferences and one-on-one meetings to ensure a quality of care delivery process for patients across the continuum.
• Accountable for maintaining patient education materials to educate patients and families on joint replacement procedures.
• Collaborate with Orthopedic Team to ensure materials are standardized and consistent with standard order sets and clinical pathways.
• Collaborate with the Nurse Manager and the education department to develop, define, and evaluate competencies to ensure staff ability to care for the orthopedic patient.
• Accountable for the TJC Disease Specific measures of the joint replacement programs and all regulatory bodies.
• Collect and presents data for dashboard reports for Joint Steering Committee Review.
• Develop data collection methods and processes for tracking and trending quality outcomes measures consistent with orthopedic patients and in collaboration with Quality, Infection Control, Physical Therapy and Case Management.
• Serve as liaison and subject matter expert to surgeons/physicians, staff, patients, families, and orthopedic nursing staff to ensure that optimal patient care is provided to the orthopedic population.

Qualifications Include:
• Bachelor of Science degree in nursing
• Three years of experience in an acute
care setting, which includes one year of
• Orthopedic experience preferred.
• Licensed as a Registered Nurse to practice in the state of Maryland; Specialty Certification within one year of hire

Work Schedule:

Days 8 am - 4pm

Pay Range:

$77,313.60 - $115,980.80

In cases where the range is displayed as a $0 amount or if the position is Per Diem (offering a fixed rate), salary discussions will occur during the screening process.
