Kidney Smart Educator, Westside Division Mesa, AZ
As a Kidney Smart Educator, you will be leading instructor-led Chronic Kidney Disease education classes throughout the Greater Phoenix Metropolitan area. Classes are held In-person, virtually and telephonically. These CKD Education classes provide the knowledge needed for class participants to take control of their health. In class, you will teach patients: How kidneys function, the common causes of chronic kidney disease (CKD), how medications, diet and nutrition work together to help keep them healthy, as well as Treatment Options/Modality Education (Transplant, Dialysis, Conservative Care)
We are looking for an internal or external teammate that can provide Kidney Smart education classes throughout division. Candidate must live local. This role works closely with local nephrology practices and the Westside/National Kidney Smart team.
We are looking for an expert CCHT or someone with a strong clinical background. Applicant must be licensed. Bilingual in Spanish is a plus but not a requirement. Travel- This role requires travel with an assigned geographical area 50-75% Hours- The schedule for this role is Monday - Friday with flexible daytime hours.
