This Lead position is an exciting opportunity to guide staff and the workflows of our coordination team! Care coordinators work with individuals of all ages to assist with coordinating their behavioral health care with internal and external care providers. The Lead Behavioral Health Care Coordinator is responsible for overseeing employees who provide direct coordination of services to patients and families as directed by the comprehensive assessment, initial service plan and/or treatment plan in support of the treatment team.Job Description
Leads the Behavioral Health Clinic with tracking, referrals, coordination of services and general non-counseling roles of case management. Utilizes all forms of communication to connect with patients and referrals. Facilitates referrals to mental health services as identified in assessment or treatment plan. Documents services provided in electronic filing system. Assists with program and employee scheduling to ensure coverage. Oversees the completion of day to day duties of employees.
Assists with referrals for economic assistance, social services, housing resources and educational services. Supports life skills advocacy, treatment follow up, disease management and educational services, including referral and linkages to long-term services and supports. Connects patients to community resources to assist in reaching successful outcomes. Is available for guidance to care coordinators in the absence of clinic management.
Qualifications
Bachelor’s degree in one of the behavioral sciences or related fields. RN or LSW preferred.Requires 2 years of experience in similar role. Requires previous experience working with/in a behavioral health clinic or medical facility.
CPR certification and AED certification required. Current valid driver’s license, and must meet all medical guidelines for Sanford Health Category I drivers in applicable locations.
