The Lead Care Management Coordinator collaborates with the patient/family, the acute multidisciplinary team, and post-acute providers to implement the patients discharge plan. The CMA also proactively escalates risks and barriers to discharge to the Case Manager; documents all discharge planning efforts in the patient's medical record in a timely manner; maintains ongoing communication with the multidisciplinary team and follows all regulatory requirements and ministry polices regarding care coordination/discharge planning.

Required Qualifications:
Coursework/Training: Completed an LVN program from an accredited school or Coursework/Training: 10 years or more care management experience or Bachelor's Degree: Completed Bachelors degree 2 years experience in health care setting (acute or post-acute) 2 years LVN experience (if LVN) 2 years experience in health care case management discharge planningWhy Join Providence?
